Patient Forms

Upon entering our office, one of our fantastic receptionists will welcome you as a member of our family. We will request that you complete our new patient forms, which can also be found by clicking here if you wish to fill them out ahead of time. This paperwork provides us with your health history and information on your condition.

  • Consultation

    Next, you will have a consultation with Dr. Ron Stoley to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Stoley to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

  • Examination

    After your consultation, Ron Stoley will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.

  • Treatment

    At the conclusion of this initial appointment, you can choose to proceed with your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.

X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.

Report of Findings

Once the information is collected and examinations are performed, Dr. Stoley will give you a detailed report of all findings and answer any questions including:

What will my treatment cost?

After reviewing your health history, goals, and examining your spine and X-rays, Dr. Stoley will discuss recommendations and notify you if your condition requires care with other providers. Dr. Stoley will provide the best treatment and wellness program for your needs.

Wellness Program

Prior to leaving, Dr. Stoley will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.

Phase One: Relief Care

Many people go to a chiropractor because they are in pain. In this first phase of care, the main goal is to reduce your symptoms. Sometimes this will require daily visits, or two to three visits per week for a time.

Most people are under the assumption that if they don't feel any pain that there is nothing wrong with them - that they are healthy. Unfortunately, pain is a very poor indicator of health. In fact, pain and other symptoms frequently only appear after a disease or other condition has become advanced.

For example, consider a cavity in your tooth. Does it hurt when it first develops or only after it has become serious? How about heart disease? Regardless of whether you are talking about cancer, heart disease, diabetes, stress or problems with the spine, pain is usually the last thing to appear. When you begin chiropractic care, pain is also the first symptom to disappear, even though much of the underlying condition remains.

​Phase Two: Corrective Care

Most chiropractors regard the elimination of symptoms as the easiest part of a persons care. If all that the chiropractor does is to reduce the pain and stop there, the chances of the condition recurring are much greater. In order to prevent a rapid recurrence of symptoms, it is necessary to continue receiving care even though your symptoms are gone.

During the correction / restorative phase of your care, you will not have to receive adjustments as often as you did during the first phase of care and, depending on your particular circumstances, you may begin doing exercises and stretches either at the center or at home to help accelerate your healing.

Do not be discouraged if you have mild flare-ups in your symptoms on occasion. This is normal. Flare-ups are bound to occur during this phase because your body has not fully healed. Depending on the severity of your injury or condition and how long you have been suffering from it, this phase of your care may last anywhere from a few months to a couple of years.

Phase Three: Wellness Care

​Once your body has fully healed, routine chiropractic care can help ensure that your physical problems do not return, and keep your body in optimal condition. Just like continuing an exercise program and eating well in order to sustain the benefits of exercise and proper diet, it is necessary to continue chiropractic care to ensure the health of your musculoskeletal system.

When you make routine chiropractic care a part of your lifestyle, you avoid many of the aches and pains that so many people suffer through, your joints will last longer and you will be able to engage in more of the activities you love.

Insurance FAQ

Here at Willow Park Village Chiropractic, Massage, and Acupuncture, we direct bill to your insurance plan whenever possible. Below is a list of the most frequently asked insurance queries and we have done our best to answer them so that you know what to expect. Please remember that every plan is different but that we will do our utmost to ensure that dealing with your insurance company is as hassle-free as possible! 

Do you direct bill to insurance companies?

Yes, we do! Every plan is different, so please make sure that your plan allows direct billing, and that you are aware of the extent of your coverage. Chiropractic, massage, and acupuncture are all designated under the paramedical section.

For chiropractic care, we direct bill to: Alberta Blue Cross, Green Shield, Great West Life, Sunlife, Standard Life, Chambers of Commerce, Maximum Benefit, and Johnson Inc. Desjardins Insurance allows billing on behalf of patients - this means you will have to pay for your treatment and be reimbursed.

For massage care, we direct bill to Alberta Blue Cross, Green Shield, Great-West Life, Sunlife, Standard Life, Chambers of Commerce, Maximum Benefit, and Johnson Inc. Please note that this depends on your therapist, so please let us know which insurance company you have benefits with so we can ensure direct billing.

For acupuncture care, we direct bill to Great West Life, Standard Life, and Johnson Inc.

If your claim is not immediately approved online, we will ask you to pay, we will print you a receipt, and we will reimburse you once we have received payment from the insurance company.

Claims that are not immediately approved online can sometimes be subject to an audit. Once the insurance company audits you, they will not send payment to our clinic and will send payment directly to you instead.

Are the massage therapists insurance compliant?

Alberta Blue Cross, Sunlife, and Manulife have implemented a requirement of a minimum of 2200 training hours for massage therapists. Call your insurance company to confirm their requirements.

All of our massage therapists have completed greater than the requisite minimum of 2200 training hours. Please note that although Dr. Le does do massage, she does not have the 2200 hour training required for most insurance companies.

Can you tell me how much coverage I have?

Unfortunately, no. Insurance companies keep that information confidential. You can, however, easily check that information for yourself by calling your insurance company, checking their website, or asking your employer. We can track the amount billed to the insurance company to give you an idea of when your limits will be reached.

What information do I need to bring in to the clinic?

All we need is your date of birth and your prescription card, which will have your ID number, group number, and section number on it. It is also helpful if you can let us know what your coverage is: what percentage of each visit is covered, the maximum dollar amount allowed annually, etc.

I have two insurance plans. How does that work?

Many people are covered under more than one insurance plan, usually because his or her spouse has a separate insurance plan. The insurance plan under which you are the INSURED MEMBER is your primary plan. If you are not the insured member on another plan, that is your secondary plan. We only bill to one insurance company at a time, so here's how we'll get you organized:

When you start out, we will direct bill to your primary plan:

If your plan covers 100% of your treatment, great! We'll bill the company when you come in, and the insurance company will pay us, so you don't need to worry about anything until your coverage reaches its limits.

If your primary plan only covers part of the treatment fee, you pay us the remaining amount and we will print you a receipt. You may then submit that receipt to your secondary insurance company for further reimbursement.

When you have reached the limits of your primary plan:

When your primary plan runs out, we will provide you with an "explanation of benefits" from the insurance company, stating that you have reached your limits. You may submit that WITH your treatment receipt to your secondary insurance company. Once you have received confirmation from your secondary insurance, let us know and we will direct bill to your secondary company.
